ACKNOWLEDGMENT OF RECEIPT OF EMPLOYEE HANDBOOK AND CODE OF CONDUCT
I acknowledge receipt of the Company’s Employee Handbook and
Code of Conduct and, if applicable, corresponding California, Illinois or New York State
Supplement to the Handbook (collectively, “Handbook”).
I understand that it is my responsibility to read and comply with the rules and policies contained in the Handbook.
The Handbook contains many important items related to my employment, including policies prohibiting discrimination,
harassment and retaliation, as well as policies on conflicts of interest, confidentiality, in-office work requirement, dress code, electronic systems usage and application monitoring, absences from work and paid time off. I am expected to abide by
all rules and policies contained in the Handbook and understand that failure to do so can
result in discipline, up to and including termination.
I understand that I should consult with my manager (as applicable), the Human Resources or the Legal Departments
regarding any questions related to the Handbook.
I further understand that, absent a contrary written contract, my employment with the Company is “at will,”
which means that it may be terminated at any time with or without cause or notice.
I understand that because the information, policies and benefits described are necessarily subject to change,
except for the Company’s policy of employment at will, the policies, benefits and the Handbook itself may be
changed or modified by the Company without notice and without a written revision of the Handbook, at the Company’s
discretion. I understand that any revised information may supersede, modify or eliminate existing policies and
benefits. I further understand that any applicable law governing my employment shall control to the extent there is a conflict with the Handbook.
